BIG FOOT takes over Sydney

International Foot and ankle surgeons descend on Sydney for the Paragon Big Foot Conference.

Many thanks to Paragon for the invitation to present on the latest foot and ankle techniques. With many international and national surgeons in attendance, it was an incredible to hear the topics and updates from surgeons all around the world. I appreciate the opportunity to be invited to give three presentations on foot and ankle replacements, approaches to the posterior malleolus and repair of the deltoid ligament in trauma. It was a fantastic meeting, great talks and ideas exchanged.

Was great to have "special guests", my own long suffering, hard working secretary @lizzie_hunter01 and podiatrist Helen from @inspire__podiatry who has helped me with my countless post op and foot and ankle patients all these years.

They came to theatre to get a glimpse of how surgeries/operating theatres function, so hopefully they can understand, empathise and work better with our patients pre and post op.

Our ankle replacements continue to perform well. This particular patient is only 8 weeks post-op and is walking with no pain. The usual rehab for an ankle replacement involves 6 weeks in a moon boot. The first 2 weeks you remain non-weight bearing after which you can start partial weight bearing for the remaining 4 weeks. In this patient’s case he required further alignment surgery to his hindfoot which kept him non-weight bearing for 6 weeks. Come talk to us if your ankle arthritis is starting to bother you.

Keyhole Bunion Surgery – Reducing the Learning Curve

Dr Gerald Yeo is now a consultant on the Stryker/Wright Medical team trying to reduce the learning curve so that other surgeons can develop their skills in keyhole bunion surgery. Minimally invasive/keyhole bunion surgery can take up to 18 months or up to 40 operations before a surgeon can be declared proficient. With the aid of Stryker, we aim to allow other surgeons visitations to observe Dr Yeo or for Dr Yeo to travel to them to minimise the length of time or number of operations required to be skilled in performing bunion surgery.

She eventually underwent an ankle reconstruction, and though she’s only five months post-op, she’s already returned to state-level lawn bowls, having made the Queensland team and just recently being made Player of the Series in Sydney.

ANKLE REPLACEMENT UPDATES

Ankle replacements are a good alternative to surgical options for long-standing ankle arthritis. The picture below shows how well this patient is mobilising after his ankle replacement.

Ankle replacements, though still relatively new in their genesis, have a 10-year survivability of 90%, with longer-term studies needed for the future. The alternative used to be an ankle fusion; therefore, preservation of ankle motion is far more attractive.

Ankle replacements still require a specific patient category. If you are elderly, healthy, have low to moderate daily activities, and have ankle arthritis with minimal deformity, you are a good candidate for an ankle replacement discussion.

Keyhole Bunion Surgery UPDATE

We have now been performing keyhole bunion surgery for over four years with good results. Compared to traditional open surgery, patients have been able to mobilise better, with minimal pain and fewer complications such as wound infection & breakdown.

Keyhole bunion surgery allows a 3D correction of the deformity, not only allowing translation but also movement to position the toe in plantarflexion or dorsiflexion. More importantly, in severe deformities, keyhole surgery allows correction of rotation, something not always possible in open surgery.

Helen has been coming to our practice for the last year now and has been absolutely invaluable to our rehabilitation of our patients both pre and post surgery. Helen is a qualified Podiatrist that specialises in all things Physio for foot and ankle pathology. She works out of her main clinic in Albany Creek which has been very central for most of my patients arriving from the Northside.

With guidance from her team, all our patients now receive immediate 2 week post op rehabilitation & functional exercises that all work to expedite their pain, swelling & post operative recovery.

Helen and her team have also been invaluable in having a all encompassing approach to foot and ankle disorders – with dry needling, acupuncture, shockwave & laser therapy as well as 3D custom orthotic inserts and gait re-training all at her finger tips.

Dr Yeo attended his 3rd ankle replacement cadaver workshop on the 19th of July at the QUT laboratory. It is always pertinent to keep up to date with new and recent advances in ankle replacement surgery, and as the saying, practice makes perfect goes. Prior practice on a cadaver allows Dr Yeo to trial and improve different skills and techniques before performing actual surgery on patients. Ankle replacement surgery is gradually increasing in numbers throughout Queensland, though minimal numbers are still being performed. If you have ankle arthritis, speak to Dr Yeo regarding whether you are a suitable candidate for an ankle replacement.

With the help of the Orthopaedic Medical Device suppliers Perios, Dr Yeo conducted a Podiatry Education evening on 8/5/2019, attended by 30 podiatrists from the North Side of Brisbane. The evening consisted of 2 talks on ankle instability and osteochondral defects of the talus. It was a very informative evening, with beneficial information transferred between both parties. Podiatrists play an essential role in the multidisciplinary approach towards foot and ankle conditions, and it was extremely useful to inform and learn from podiatrists their thoughts and approaches to the above conditions. We are hoping to conduct a 2nd Podiatry evening soon.

Brisbane Orthopaedic Specialist Services was proud to sponsor a team that entered The Wild Women On Top Coastrek.

What is Coastrek?

The Coastrek is a 30 and 60-kilometre team trekking challenge to raise money to restore sight.

Trekkers fundraise and raise money to help eliminate avoidable blindness by walking along some of Australia’s most beautiful coastlines.

Sydney Coastrek is an annual event supporting the Fred Hollows Foundation and allows the Foundation to continue its vital work to end avoidable blindness.

Coastrek Route

The trek, starting at sunrise, took our team of four “Angels” through pristine national parks, secret coves, stunning beaches, Sydney’s Harbour Bridge, and past other iconic landmarks.

The trek was 30 km from Rushcutters Bay to Tania Park near Manly Beach, and thankfully, we couldn’t have asked for better weather.

The four girls spent 12 weeks training with regular Sunday morning long-distance walks along the Brisbane River. This did not really prepare us very well for the ups and downs of a coastal walk, and our tracking devices registered around 36 kilometres on the day.

Completing the trek and raising money for this worthy cause was a great achievement.
