Bilateral hallux valgus corrections with hammer toe corrections. The right side was quite severe and has a dislocated 2nd toe MTPJ despite the appearances not looking too bad, so this was decompressed with an osteotomy while the left wasn't too bad so this underwent a plantar plate repair. #bunions #hammertoe #podiatry #physiotherapy #footandankle

The importance of recognizing a stage 4 flat foot with deltoid and incompetency. Chronic medioclear space widening from a mist deltoid rupture where only the syndesmosis was repaired. Patient developed chronic and stability with progressive flat foot. Calcaneal osteotomy with Cotton osteotomy and LARS reconstruction of deltoid to restore stability and correct the flat foot. #flatfoot #deltoid #footandankle #podiatry #physiotherapy

This patient has a very rare avulsion fracture of the posterior tuberosity of the calcaneus that was left untreated. As a result they complained of a broadened heel and aching around the Achilles as it's entire attachment had detached. Surgery involved exposing the fragment posteriorly followed by the careful dissection of the Achilles tendon away from the fragments. You can see in one of the photos the multiple fragments that were dissected free to reduce the width of the heel. Proceeded with a standard suture anger repair of the Achilles down to the calcaneus augmented it with a dermal patch which I find gives better creep and therefore is less stiff than the normal LARS patch I would use. #achilles #footandankle #physiotherapy #podiatry #rupture

Not all bunions can be managed through percutaneous or minimally invasive techniques. This patient in particular and arthritis of the 2nd TMTJ and instability of the first Ray. Complete rupture of the tibialis anterior tendon 4 months ago. The telltale lump in 1st picture is retraction of the tendon under the extensor retinaculum. The second picture showed a defect with retraction of almost 7cm, which we will unable to bridge by a end to end repair so an allograft tibialis anterior tendon was used to bridge the defect. Important to have good repair of the extensor retinaculum back over the tendon to prevent bow stringing. This time a deltoid using a LARS ligament. Two anchors are placed for repair of the deltoid initially then the LARS is placed in the medial malleolus to create a two-tailed graft. 2 tunnels placed respectively in the talus and sustentaculum to recreate the superficial and deep deltoid ligaments. The remaining anchors are then passed through the deltoid ligament and the graft tightened with the ankle in neutral. #deltoid #reconstruction #footandankle #podiatry #physiotherapy

Shifted the calcaneus into varus for better Achilles alignment. The widening between the base of the metatarsals consistent with a Lisfranc injury. This ligament is important for the stability of the midfoot and the arch. Is patient had significant hypermobility and despite a previous lateral ligament stabilization with suture brace they progressively loosened and became unstable again. I had already demonstrated the LARS reconstruction technique but just wanted to show a better picture of the location of the tunnels used on x-ray. The lateral demonstrates position of the CFL tunnel and the pre-existing anchor in talus used for the suture brace, so had to go slightly more anterior for the new ATFL attachment. Achilles avulsion from the attachment to the calcaneus, rare but devastating injury. The principal remains that the tendon needs reattachment to bone, however this is not always that easy, and so as reinforcement, a LARS graft was utilized as an only patch to encourage healing to bone, while the tendon was repaired using anchors through a standard double row speed bridge type repair. #achilles #rupture #footandankle #physiotherapy #podiatry

The rare case where an MIS bunion can progress to a non-union. The initial metal work required removal and the non-union site was debrided back to healthy bone and multiple drill passes to stimulate new bone. A allograft cancellous buying block was used to fill the defect and stop the first Ray from over shortening. The central hole was drilled and filled with auto graft from the heel. New year, new ankle replacement. The x-rays were deceptive, showing only relatively mild valgus, but the plan showed otherwise, confirming a 13 valgus tilt. Ankle fusions can give very good pain relief however adjacent joint arthritis can occur which led to us taking down this ankle fusion and converting to an ankle replacement. Sometimes ankles are chronically unstable and with either a previous failed reconstruction or in particularly the ligamentous lax, an augmentation with LARS is required. The initial tunnel is placed in the calcaneus with a stab incision, held with an interference screw. Through the standard lateral incision the ligaments are taken down and repaired back with 2x anchors Brostrom style. A tunnel is drilled through the fibula in an AP direction, after which the LARS is retrieved passing under the peroneals and through the fibula tunnel from PA. This is similarly held with an interference screw. Finally the tunnel for the ATFL is found under II guidance and the LARS passed through lateral to medial, held with one last interference screw. #reconstruction #unstable #lars #recon #footandankle #podiatry #physiotherapy #instability

Normally I would place 2x syndesmosis buttons, but because the deltoid was repaired with anchors, I managed to get a really good reduction of the joint space, so managed to get away with 1x button just to make sure the syndesmosis heals. First use of the Paragon28 Reflex syndesmosis stabilisation system in Australia! Thank you Paragon28 for allowing me the ability to use the Reflex system for this post traumatic ankle arthritis patient. The patient had a previous syndesmosis injury not stabilised hence leading to arthritis. I knew I wanted to insert a suture button but it would compromise the insertion of the Inbone stem. Typical Weber B fracture ankle with talar shift/increased medial clear space. More and more I've been opening and repairing the deltoid medially. I know the evidence is still out there on it's outcomes, but it certainly provides significant stability to the ankle, hence the use of only 1x suture button instead of my usual 2. Rare case of a chronic FHL tendon rupture to the great toe. 1st pic showing a large gap approximately 6cm after retraction. This required a plantaris tendon allograft to bridge the gap. 6 months post op and able to begin a single heel raise, with a well healed wound. Still minimal flexion at the IPJ, but this is unfortunately what literature reports as well. Another flat foot reconstruction, the heel wasn't too valgus, but noted abduction and talonavicular uncoverage, so an Evans was performed instead along with a Cotton osteotomy and TAL. Open fixation of all 3 components, in particular use of the postero-medial approach rather than a standard posterolateral, which can decrease incidence of peroneal artery injury causing stiffness to the ankle via necrosis of the FHL muscle. Re-rupture of the Achilles tendon can be increased up to the first 3 months from initial tear. This patient had an initial repair by another surgeon that re-ruptured soon after coming out of the moon boot. The repair was taken down. Rupture of the Achilles avulsed from the heel, rarer type of injury but still significant, and not to be treated as per a regular tear of the Achilles. In this case the tendon was retracted almost 6.5cm, making it difficult to repair back to its origin. The Haglunds of the calcaneus was debrided to create a bleeding surface to repair back to. The white plugs are Tetrous plugs, a biologic plug to optimize enthesis healing to bone. An additional Lars ligament was used to augment the repair, and a Speed bridge repair of the tendon back to bone with a double row anchor repair, placed in the Tetrous plugs to encourage growth/healing back to bone. #achilles #rupture #plugs #repair #haglunds #footandanklesurgery #podiatry #physiotherapy

This patient had a fixed plantarflexion of the foot, and had been walking on her tiptoes without being able to put her heel to the ground for years. At time of surgery most of the stiffness was actually in her ankle, nonetheless we lengthened her Achilles, and did a triple fusion, with a closing wedge osteotomy performed through the talonavicular and calcaneocuboid joints to get more dorsiflexion. Still not perfect, but will do for now. When she's fused we will do a 2nd stage total ankle replacement to get more movement from the ankle with a posterior capsular release as well. #2stage #stiff #anklereplacement #triple #footandanklesugery #podiatry #physiotherapy

This patient had the worst luck in sustaining not only 1 but simultaneous, bilateral Achilles tendon ruptures. Quite a rare occurrence, both were also repaired simultaneously through mini incisions to expedite recovery and mobility. Normally I would place these in plasters for the 1st 2 weeks, but to allow mobility, the dclecision was made to allow immediate stand transfers in bilateral Moonboots with large heel wedges/blocks. #badluck #simultaneous #bilateral #achilles #footandanklesurgery #physiotherapy #podiatry

As much as I love ankle replacements sometimes there are still certain indications for an ankle fusion. Surprisingly patient satisfaction is equal to an ankle replacement and rate of complications are reduced. A high ankle sprain or syndesmosis injury can be easily missed as a simple ankle sprain, but have consequences further down the track leading to post traumatic arthritis. This is an arthroscopy of the ankle showing a probe inserted in the syndesmosis. As I twizzle the probe, the fibula is seen moving away from the talus, displaying instability of the syndesmosis. The difference in instability to the ankle before (above) with an anterior drawer test and after (below) where we have repaired the lateral ligaments to provide stability. A low lying peroneal muscle belly can be a genetic source of pain and can be associated with tears of the peroneal tendons, as seen in this patient. In this case the peroneal muscle extended way past the fibula groove down to the level of the peroneal tubercle. Bilateral bunion corrections done minimally invasive. Weight bear immediately for 6 weeks and then back in normal shoes. Importance is to recognise the 3D nature of rotation in bunions, which requires correction to prevent recurrence. This patient came not long after a pain free ankle replacement saying his ankle suddenly became swollen and sore. After multiple investigations including scans and samples he was taken to theater and discovered the implant peg had actually broken off. You can see the metallosis in the wound causing loosening of the talar component. Revised to a stemmed Inbone implant. How technology has changed. This patient had a similar issue on his contralateral ankle, and we opted to do a hind foot fusion nail only 2 years ago.
